Anyone have a tech inspection form they can send me?

I don't know if asking for a tech form in a non-tech forum is technically correct, but I'm doing it anyway.

So does anyone have a tech inspection form they'd be willing to donate to the cause. NW Fest is coming up quick, and I'm too lazy to make a form. Something along the lines of the inspection form used at the Fall Fling last year would be great.
