I bet the slugs diverge from running parallel due to the shock wave from each bullet pushing the other away.

Chances are there aren't even 3 holes in the target,.. that's why. 'Probably wasn't even firing at a target.

Both barrels are probably capable of one-hole groups at 25 yards,.. just not the same hole. Regulating 2 barrels to fire to the same point of aim is as much an art as an engineering challenge.
