Serpentine belt jump one throat

This normally means that one of the pulleys is not lined up. First off, check that the harmonic balancer is in good shape and that the rubber isolator is not spitting out and causing it to wobble or shift. Beyond that, I have had issues with the power steering brackets not staying straight. This can be caused by a missing bolt on the back of the PS pump or other missing/loose bolts.

Just like the harmonic balancer, the water pump can have a bad bearing allowing the shaft to wobble or the alternator could be missing the rear bolt.

Double check the belt routing.
 
Take a close look at all the pulleys. They all should be pointing exactly forward and not at any angle. To verify, try laying a ruler/straight edge across each pulley. Especially look at the power steering and water pump. The idlers also can be tilted.

For the moment, try and reverse the belt 180 degrees and see if things improve. You could have a belt that has a defective inner cord.
