Clutch disengagement issues, any ideas?

Posting this for a friend of mine, I can't figure it out. Here's the info...

Vehicle is a 1995 Cherokee sport 4.0l 5 speed (AX-15) Blew a tranny and needed to replace it used a 91 5 Speed out of a Cherokee. Had a pilot bushing made by a friend who works in a machine shop. After we got the tranny back in the clutch would not disengage.

There is clutch pressure, fluid in the system and has been bled. We haven't pulled the tranny back out again in hopes it may be something simpler. let me know if you guys have any ideas on what we can check before we pull the tranny again.
 
Is the 95 an internal or external slave? If it's external, the fork or linkage inside the bell might have come off. Not sure on the pilot bushing, but mine has a pilot bearing.
