d44 yoke u-bolt conversion

I must be having a brain fart today, does anyone have the link to the manufacturers selling the yukon d44 yoke that accepts and comes with u-bolts and a 1310 u-joint?

I had it bookmarked somewhere, but now I can't find it :\

I tried searching because I found it in a post here originally, but i'm not having any luck.
