Mis-assembled rear brakes

My newly acquired 99 has been experiencing the dreaded rear-wheel lockup in this recent wet weather, so I opened up the rear brakes expecting to see rusty adjusters and worn out brakes. Instead I found a nice recent brake job with all new parts, even new drums, but whoever had put it together did not get the little adjuster cable guides properly seated in their holes in the shoes, so it didn't adjust because the cables were effectively too long. The guides are set into the hole in the shoe loosely, and the top spring holds them in place, but if they're not in all the way they will still be held by the spring from falling out, and it's not obvious at a glance what is wrong. If your adjusters aren't adjusting it's something to look out for.
 
The most common screwup I have seen is swapping the leading and trailing shoes.
