O2 Sensor replacement

Hey there,
I ran a diagnostic scanner on my 97 4.0 and got error code P0141 which is 'O2 Sensor Heater Circuit Malfuncion, Bank 1 Sensor 2' and code P0138 'Sensor Circuit High Volts, Bank 1 Snesor 2'....my question is a) will replacing the sensor fix this and b) is Bank 1 Sensor 2 the downstream sensor (is there a difference in the sensor itself, or just location)?

Now you need to find out if the heater in getting proper voltage, could also be a bad o2 relay.
