crapy idle/stalling out when cold

for about the first 5 min my rig (2.5 4cyl) is running, it has a problem with a really low idle (sounds like its running on 2 holes) then some times it gets really high. then when im driving it acts like its running out of fuel. it only cuts out for a second then its fine. after about 5 min its fine and wont do it untill i shut it off and drive it a little later. any ideas? thanks for the help.
