Tranny problems

ok so i started having tranny problems about a month ago, i changed the fluid and all that good stuff and it seemed fine. Today on the way home from work, it started again. After coming to a complete stop and trying to accelerate, the RPM's shoot up to 2500 before the car starts to move. Once it gets going aroung 60mph it wont shift and it will be around 3000rpm...
Bottom line is my tranny done ?

I have around 150Kmiles
