Starting strangeness

Just over the last couple of days I've noticed an odd thing happening when starting up.

If I don't give the engine a touch of gas on starting it doesn't immediately fire up. On the second attempt it doesn't fire at all, and there's been a funny knock/ slap sound as the starter turns the engine over. Of course, it fires up fine after that, and behaves normally.

I'm assuming that this is the pistons slapping due to the cold weather. It never did it last winter, which makes me wonder, as I've not put that much mileage onto it (10k max), although it is an old vehicle ('93 HO, 156k). One other thing is that it's always smelled hot, and might have a little leak in the cooling system somewhere which I still have to track down. Could this be connected somehow?

Is this something I should start to be worried about?
