D60-ISU Yoke Part Number?

I think there are a couple people on here running the 60-ISU rear axle out of the E250/E350 vans. I'm looking for the part number for the yoke. Specifically, the 99-04 (?) version with the flange and bolt on yoke (same style as 8.8). I can't find it with google, and the dana expert site gives the companion flange part number, but not the yoke.
