Prothane uca install axle end...thrust washer?

I'll do my best to explain this so bear with me.
I need to see pics of the proper way to install prothane poly uca bushings at the axle end when rubicon express control arms are used.

The directions say to remove stock rubber part and leave metal shell which I have done.The prothane bushings were a loose fit into the shell and I was able to push them in by hand....the kit comes with large poly thrust washers that I assumed were only used with stock control arms to I left them out now I'm starting to thing they were supposed to be used because my passenger side bushing has started to pop out of the mount.

Anyone have pics of these bushings installed at the axle end using aftermarket control arms like rubicons??
Do the side thrust washers need to be used?How are they orientated??
