Jerky clutch

Allright, this might be hard to explain right so bare with me..

I have a '90 4.0 with an AX15. It shifts kinda ok and doesn't make any noise.
However it's kinda rough and jerky when engaging the clutch. Feels like it's either on or off.. Can't make smooth gear changes.
Starting from zero on an incline is kinda hard and takes some revving to avoid jumping forwards and possibly stalling. Makes it very unpleasant on tight trails.
Also i notice when backing up that i get vibrations, i assume it's from the tranny.
Do i just need to get a new clutch or could the problem be hydraulic?

I've had that happen, turned out I had air trapped in the hydraulic line. Try bleeding it like you would a brake line.

I ended up finding a leak in the slave cylinder that was allowing air to be pulled into the system.
 
Kind of sticky when the pedal is part way out?
Tend to grab, let go, grab, let got which causes the entier vehicle/drive train to shudder.
If so, it usually is a sign that the clutch disk and pressure plate are heading South.
Sometimes with a bit of finess, they could be made to last a long time.
